If it's a General MIDI game, it will be translated to MT-32. If not, ScummVM will select the default device for the game, which should be Adlib or PCjr then. CD Audio has nothing to do with MIDI. In fact the Monkey enhanced CD plays CD audio and has some Adlib SFX tracks in addition to that.
Mixed mode plays MT-32 or General MIDI tracks if available and Adlib tracks if not. Monkey 2 and Indy 4 have many cases where there is only an Adlib version for certain SFX which weren't be played at all when using MT-32 with the original executable.
