Might not be the solution for your problem... However when I started my game after previously running fullscreen, it would only do show as a window. Same as you - setting was set to fullscreen. Solved by hitting alt +enter - as in the shortcommand for for fullscreen in alot of other stuff. Anyhow thought it might be worth sharing so you could give it a try.
